If your GP believes you would benefit from a Home Medicine Review and you agree, they will refer you to a community pharmacy of your choice and share relevant medical information with the pharmacist.
STEP 2: HOME VISIT APPOINTMENT
The pharmacist will arrange a convenient time to visit you, usually in your own home where you're most comfortable.
STEP 3: COMPREHENSIVE MEDICINE REVIEW
During the visit, the pharmacist will review all your medicines, including prescriptions, over-the-counter products, and supplements. They will also discuss how you take them, address any difficulties, and check for potential interactions.
STEP 4: PROFESSIONAL CONSULTATION
The pharmacist discusses their findings with your GP, who will then speak with you about any recommended changes to optimize your medication management.
